SNAP-IV (Long form)

The SNAP-IV Teacher and Parent Rating Scale (Swanson; University of California, Irvine) in its long 90-item form is an informant rating — completed by a parent or teacher, not the child — of ADHD and Oppositional Defiant Disorder symptoms plus related classroom and comorbidity indexes. It revises the earlier Swanson, Nolan and Pelham (SNAP) Questionnaire (Swanson et al., 1983), with items drawn from the DSM-IV criteria for ADHD and ODD, the Conners Index (Conners, 1968), and the IOWA Conners (Loney & Milich, 1985).

Each item is rated 0–3 (Not At All to Very Much), and every subscale score is the average rating per item — the sum of its items divided by their number — exactly as on the source scoring page. The source's tentative 5% cutoffs differ by rater (teacher vs parent) and are applied per subscale; the rater-appropriate thresholds are shown with the score bands. The source defines no cutoff for the I/O, A/D, Conners Index, or SKAMP subscales, so those are reported without thresholds.

Scoring. Scoring (source scoring page): each subscale score is the sum of its item scores divided by the number of items — the Average Rating-Per-Item on the 0–3 scale (Not At All=0, Just A Little=1, Quite A Bit=2, Very Much=3). Worked example on the source page: ADHD-Inattention total 18 / 9 items = 2.0. Tentative 5% cutoffs (Teacher / Parent): ADHD-Inattention 2.56 / 1.78; ADHD-Hyperactivity–Impulsivity 1.78 / 1.44; ADHD-Combined 2.00 / 1.67; ODD 1.38 / 1.88. The source defines no cutoff for the I/O, A/D, Conners Index, or SKAMP subscales.
